MN law M.S. 121A.15 requires written proof of required vaccinations for children in grades K-12. However if a child has a medical reason or if his/her parents are conscientiously opposed to any or all of the shots, a legal exemption is available. To file an exemption you must fill out this form and return it to the School Nurse. Conscientious objections to immunizations need to be notarized.
